Guideline on targets for solute and fluid removal in adult patients on chronic peritoneal dialysis.

چکیده
The previous decade saw the generation of a series of consensus and evidence-based guidelines for the optimal management of dialysis patients. The majority of these have emanated from national and regional committees. There is not always agreement among these documents; part of this disparity may have to do with the nuances of dialysis practice in different parts of the world. The International Society for Peritoneal Dialysis (ISPD) has commissioned a working group with representation from Asia, Australia, Europe, and North America to formulate a series of recommendations concerning the delivery of adequate peritoneal dialysis (PD). It is the hope of the authors that the comments and recommendations presented here are relevant and applicable to those worldwide who manage patients on PD, and that they are readable, precise, and concise. These recommendations have been approved by the Standards and Education Committee of the ISPD.

Are adequacy targets for peritoneal dialysis meaningful?
p eritoneal dialysis has undergone considerable change since it was first used in managing end stage renal disease (ESRD) patients in the early 1960s. Its use then was predominantly as a holding procedure for hemodialysis, and as such, it was used in a very limited number of patients. With the introduction of continuous ambulatory peritoneal dialysis (CAPD) in 1976, the use ofPD has increased d...
متن کاملIncreased peritoneal permeability is associated with decreased fluid and small-solute removal and higher mortality in CAPD patients.
BACKGROUND Recent studies suggest that increased peritoneal membrane permeability is associated with higher morbidity and mortality in peritoneal dialysis patients. It is not known, however, whether the difference in clinical outcome among different peritoneal transport groups is due to differences in peritoneal fluid and solute removal. In the present study, we compared the peritoneal fluid an...
متن کاملRisk of extracellular volume expansion in long-term peritoneal dialysis.
Although direct evidence is unavailable, indirect evidence strongly suggests that the risk of extracellular (EC) volume expansion increases with long-term peritoneal dialysis (PD). Long-term PD patients routinely develop loss of residual renal function (RRF) and often develop increased rates of peritoneal solute transport. Loss of RRF is associated with hypervolemia and increased risk of death....
متن کاملExtended Peritoneal Dialysis and Renal Recovery in HIV-Infected Patients with Prolonged AKI: A Report of 2 Cases
Peritoneal dialysis (PD) has recently been established as a treatment option for renal replacement therapy (RRT) in patients with acute kidney injury (AKI). Its efficacy in providing fluid and small solute removal has also been demonstrated in clinical trials and is equivalent to hemodialysis (HD). However, effect of RRT modality on renal recovery after AKI remains a controversy. Moreover, the ...
متن کاملEffects of chondroitin sulphate on fluid and solute transport during peritoneal dialysis in rats.
The effect of chondroitin sulphate (CS) on peritoneal fluid and solute transport was studied in rats undergoing peritoneal dialysis. In the presence of CS, net ultrafiltration increased, while absorption of glucose and horseradish peroxidase from the peritoneal cavity decreased. Albumin, used instead of CS, did not modify either fluid or solute transport. In in vitro experiments on isolated rab...
